小针刀治疗屈指肌腱鞘炎203例报告许才祥    丁勇    杨志刚 （鄂尔多斯市中心医院康复科  内蒙古自治区  中国 017000） 【摘要】目的：报告203例屈指肌腱鞘炎的针刀治疗效果；方法：针刀闭合术；结果：203例382指一次治愈在95%以上。结论：针刀闭合术是屈指肌腱鞘炎临床治疗的首选方法。     The acupotome were cured 203 cases of the tendon sheath inflammation of the bend finger muscle．Xu Cai-xiang，Ding-yong，Yang Zhi-gang（The recover department of the center hospital of Erduos city，Inner Mongolia，China，017000）【Abstract】Objective To report the curative effect that the acupotome were cured 203 cases of the tendon sheath inflammation of the bend finger muscle；Methods  The acupotomy；Results  The restore health rate are over 95%；Conclusion  It is first meyhod that the acupotome curing the tendon sheath inflammation of the bend finger muscle on the clinic．【Key Words】tendon sheath inflammation of the bend finger muscle   acupotomy 1 一般资料从1998.6—2003.3共收治狭窄性腱鞘炎患者203例，其中女性191例，男性12例，第一拇指167例，食指21例，中指13例，右侧手指181例，左侧手指22例。（可能与右手活动较灵活有关系，且女性多与职业，手工业操作有关。）2 诊断标准2.1患者大多为手的掌侧掌横纹处有压痛，有明显的结节，指间关节屈曲时，按压结节处，不能伸直，指间关节活动有弹响，X线提示骨质结构关节结构均正常。2.2无明显的外伤史，多见于手工纺织工人，女性患者较多。3 治疗3.1常规消毒、辅无菌小孔巾，痛点局麻后，针刀于掌横纹处垂直进针切开，松解数次，过度屈伸手指数次后包扎。3.2或用带勾的小针刀更好。优点：松解彻底，但损伤较针刀大，4 结果203例患者382指一次治愈95%以上，个别患者需2——3次后痊愈。5 体会针刀治疗屈指肌腱鞘炎，可直接切开其狭窄、挛缩的腱鞘及关节囊，有效地解除局部的疼痛、弹响及 功能障碍，无伤口，损伤小，疗效好，完全可取代外科手术切开及封闭治疗。
